Use this factsheet to reflect on ways to help healthcare providers recognize that people with disabilities are in every community and should be seen in their practice. This reflection sheet may be used to support conversations on healthcare transition for youth with disabilities, dedicating time to review and update health records or “Own My Health Record,” use of telehealth, and access to timely health services like the seasonal flu shot.
